Dental services include a full range of general dentistry for children and adults, including cleanings, x-rays, fillings, oral surgery, prosthodontics and periodontic services. For the convenience of residents with busy schedules, these clinics provide day, evening and weekend hours. Following are the addresses and phone numbers of the dental clinics. Belvidere Medical Building 2400 Belvidere Road - Waukegan (847) 377-8410 North Chicago Health Center 2215 14th St - North Chicago (847) 984-5230 Mid Lakes Health Center 224 Clarendon Ave - Round Lake Beach - (847) 984-5130 North Shore Health Center 1840 Green Bay Rd - Highland Park (847) 984-5330 All Lake County residents are eligible for these services. Patients are charged on a sliding fee scale based on a patient's ability to pay and family size according to the most recent federal poverty guidelines.
